Skates come in widths as well as lengths. A standard width on a skate is D width or sometimes called R for regular, similar to a shoe. Some skates, depending on the manufacturer offer wider widths like E or EE. If you are able to go somewhere and get fitted for a skate they will be able to tell you what you need.

Yes he would still be in a Youth sized skate. Most skates fit a size to a size and half down from regular shoe size.
